and Family Advisory Council
are seeking patients or family caregivers to join our Patient and Family Advisory Council (PFAC).
participation, along with 4-6 family caregivers and 2-3 staff members will assist the practice with improvements in care,
operational efficiencies as well as reviewing new or expanded services for the practice.
The council will meet bi-annually
(spring and fall) for a one hour onsite meeting to discuss relevant practice topics.
